Application Process

Only qualified professionals who meet and comply with minimum qualifications will be eligible to be part of Cornerstones of Care Service and Placement Provider Networks.

CLINICIAN

Minimum Qualifications:

• Current license or provisional license (LPC, PLPC, LCSW, PLSW, etc.) in professional counseling social work, or psychology from the State of Missouri
• Demonstrated competence in providing clinical services to children & families
• Absence of criminal acts of violence against a person
• Absence of preponderance of evidence findings (probable cause) of Child Abuse/Neglect
• Professional Liability Insurance
• Internet access

Application Requirements:

• Provider Application-Clinician
• Submission of graduate education diploma(s) from an accredited masters or doctorate program in counseling, social work, psychology, or other related field
• Current Resume or Vitae
• Copy of current professional license
• Copy of Professional Liability Insurance, showing single event coverage along w/ expiration date
• Kansas-Criminal & Child Abuse/Neglect Permission Form
• Missouri-Criminal & Child Abuse/Neglect Permission Form
• Evidence of certification in any applicable specialty areas

PARAPROFESSIONAL

Minimum Qualifications:

• Demonstrated competence in providing services to children & families
• Absence of criminal acts of violence against a person
• Absence of preponderance of evidence findings (probable cause) of Child Abuse/Neglect
  
• Internet access

Application Requirements:

• Provider Application-Paraprofessional
• Submission of education diploma(s)
• Current Resume or Vitae
• Copy of Professional Liability Insurance, showing single event coverage along w/ expiration date
• Kansas-Criminal & Child Abuse/Neglect Permission Form
• Missouri-Criminal & Child Abuse/Neglect Permission Form
• Evidence of certification in any applicable specialty areas
• Copy of current Driver’s License and Proof of Auto Insurance

FOSTER PARENT

Minimum Qualifications:

• Demonstrated competence in providing services to children & families
• Current placement license in good standing for type foster home or residential facility contracted
• Absence of criminal acts of violence against a person
• Absence of preponderance of evidence findings (probable cause) of Child Abuse/Neglect

Application Requirements:

• Provider Application-Foster Parent
  
• Copy of foster parent or residential placement license
  
• Copy of current Driver’s License and Proof of Auto Insurance
